S5:E3 Hammermill

During Holly's ethics seminar, Meredith reveals that for the past six years she had been sleeping with Bruce Myers, the Scranton representative for Hammermill, in exchange for discounts and Outback Steakhouse coupons. Staples had an exclusive hold on Hammermill products up until S3:E2, when Michael announced to Jan, Josh, Jim, and Dwight that he had, for the first time, gotten Hammermill to break their exclusive ties with Staples. Less than six years passed between S3:E2 and S5:E3, meaning Meredith could not have been receiving Hammermill discounts for the duration of her relationship with Myers.

S5:E1;2 Hollis Flax

Holly's real name is Hollis. While looking over Holly's shoulder, we can see a nameplate on her desk that reads Hollis "Holly" P. Flax. 

S5:E1;1 What Did Meredith Catch On The Boat?

During a weigh-in, we can see Meredith's face covered in some kind of horrible redness. Either burns, a rash, disease, etc. She is rather disfigured, yet no mention of it is ever made, and it plays into no scene either before or after this. Confirmed Cause: Deleted Scene A scene was deleted in which Meredith explains that "These guys took me out on a fishing boat... I don't think I caught anything." In the next scene Michael says to Meredith "Hey! Your face is... almost back to normal."

S4:E14;2 Andy's Parents Were Replaced

During Toby's going away party, Andy gets on stage to propose to Angela. He says he can't think of a better time, with the music playing, all his friends around, and his parents being there. The camera pans to his mother and father, who are two completely different actors than who play his parents in later episodes in following seasons. In this shot we see his parents seem very happy to be there, and happy to be given an introduction by Andy from the stage. His 'real' parents as we know them later on are ashamed of Andy and his antics, and would not have reacted the same way.

S4:E14;1 Raccoon Hazing

When Meredith, Dwight, and Mose break into Holly's car to leave a raccoon inside, Meredith inserts a crowbar of sorts between the door and the window to disable the lock. We can clearly see that the driver's side window had been open the whole time.

S4:E13 Nick Used To Be A Designer

At the high school job fair, Pam approaches a Graphic Design company's booth and chats with the representative about the job. The representative is Nick "Glasses", who later becomes Dunder Mifflin's IT Specialist, replacing Sadiq "Turban" in later seasons. Nick quits Dunder Mifflin in season 6 to help inner city kids with computers. Possible Cause: Graphic Design is a dying career path, thus Nick had to become an IT specialist.

S4:E11 Hop The Fence And Jog Home

While locked in the Dunder Mifflin office park and after Toby awkwardly caresses Pam's knee in the lobby, he announces he's moving to Costa Rica and says "I'm just gonna hop the fence and jog home then." We can clearly see a stunt double in place of Toby (Paul Lieberstein) scaling the fence, jumping over, and jogging away.
